Navigating Senior Care
It can be hard to find the resources you need for yourself or a loved one. Our experienced care managers can help you figure out what you need and how to access services both within the Council on Aging Martin County and the greater community.
Care Management Services
- Comprehensive in-home assessments
- Professional advice about available resources for seniors and their families
- Development and implementation of a care plan to improve clients' health and well-being
- Identification and management of support services such as in-home care and nutrition
- Assistance with bill paying and insurance/medical claims processing
- Long-term care planning
- Caregiver support
- Assistance with family dynamics
The cost of the service varies depending on the type and frequency of services.
We accept payment from the following: Veterans' Benefits, the Medicare Long-Term Care program or programs funded by the Area Agency on Aging, and Martin County Aids and Grants.
We also accept private pay and donations. Financial assistance is available for qualifying seniors.
Senior Services Navigator
A Senior Services Navigator is available at the Kane Center for walk-ins and callers seeking assistance about senior-related issues and resources.
- Get help with referrals for community-based services, financial assistance, and placement on the waiting list for government-funded services.
- Access Council on Aging programs like Meals on Wheels, case management, adult day services and senior-focused primary care.
- Available to offer support and guidance to stabilize and/or diffuse crisis situations.
