Bringing Care and Support Straight to You

Wherever you are in Martin County, the Council on Aging is here for you! If you can't get to us, we'll come to you.

Our Elder Mobile Outreach Bus brings services directly to vulnerable seniors right in their neighborhoods and links them with resources to help them stay independent in their own homes.

Our Navigators Can Help With:

  • Securing affordable housing
  • Referrals to Meals on Wheels
  • Referrals to the Adult Day Club at the Kane Center
  • Enrollment in the GUIDE program for people with dementia and their caregivers
  • Medical consultation and screening from the Day Primary Care Center
  • Assistance with everything from unpaid utility bills to loneliness and isolation
